A 5-month-old girl died Monday while under supervision of staff at a Montgomery County daycare, CBS3 is reporting.

Police told the station that the baby was rushed to nearby Chestnut Hill Hospital, where she was pronounced dead.

The Pennsylvania Department of Human Services commenced an investigation immediately, which revealed more than a dozen violations, and on Wednesday closed the school, Wyndmoor Learning Center, at 1400 East Willow Grove Ave. in Springfield Township. The school, also known as the Wyndmoor Montessori School, opened in 1963.

6ABC reports that investigators say they reviewed surveillance video during an inspection of the facility that allegedly showed a staffer putting a sheet over the child's face. It's not clear what happened next, but paramedics were called and the staffer was allegedly seen trying to hide the sheet.

CBS3 reporter Cherri Gregg interviewed the mother of three children who attended the school:

"I would never expect anything like that happening at this school," said Angela Curtis, who learned the school would close via text message. "The kids loved this school, they are heartbroken. It's a great facility, wonderful teachers, staff .... it was a major shock."
